OP don't fret about the gains too much. They will go but when you're back to it your old muscle will come back very quickly. Muscle memory. I have lost mass plenty of times and it comes back very easily once I return to adequate nutrition and lifting frequency.

i took three years off lifting, with only very very brief periods of discipline sprinkled in. i've been lifting since the beginning of summer and my strength now is greater than when i quit. i'm 35 so its not like i've got youth on my side either,.
i broke my hand punching a wall and immediately after it healed I got a urethroplasty to treat a urethral stricture that's been haunting me for the past 12 years, so just been crippled for the past 3 months
How do I transition from weightlifting to cardio? It's all I can do now that I have tendonitis in both elbows and am due for wrist surgury to fix a fracture with a 4 month recovery very soon.
Biking does wonders to maintain leg strength (hamstrings might fall a little depending on technique). Even just steady state is good, try HIIT if you like.
